* jtagmkII.c: Many fixes for Xmega devices.
* jtagmkII_private.h: Add various new constants required for
Xmega devices.

ATXMEGA256A1, ATXMEGA64A3, ATXMEGA128A3, ATXMEGA192A3,
ATXMEGA256A3, ATXMEGA256A3B, ATXMEGA16A4, ATXMEGA32A4,
ATXMEGA64A4, ATXMEGA128A4
* avr.c (avr_read, avr_write): Add more names for (Xmega)
memory areas that require paged operation.
